Notice of Understanding and Agreement

I understand that Amalia Ann Samaraweera and her partner Scott Anthony Hutchinson are not medical doctors or psychologists. They are alternative health care consultants.

Amalia Ann Samaraweera holds a certificate of completion of training in Computerized Meridian Stress Analysis from Qest4 and Natural Solutions. She also holds a certificate in Auriculotherapy from the Institute of Bioenergetic Medicine.

Scott Anthony Hutchinson holds a certificate of completion of training in Computerized Meridian Stress Analysis from Qest4 and Natural Solutions. He also holds a certificate in Auriculotherapy from the Institute of Bioenergetic Medicine. In addition, he holds a certificate of completion in Craniosacral Therapy CST1 from Upledger Institute.

I, the client, am not consulting with Abundant Natural Wellness Clinic LLC, either in person, by phone, or through written or electronic media, for the purpose of medical diagnosis, treatment, or psychological evaluation. I understand that any reports provided are non-diagnostic, and will never indicate the presence of disease, nutritional deficiencies, hormone imbalances, conditions, etc. This information is intended for educational use only. Any supplements taken by the client are their own choice and the client assumes all associated risks, and therefore should be reviewed with a qualified licensed medical healthcare professional.

The services performed are at all times restricted to helping me gain a better understanding of my level of bioenergetic health so that I will have a greater self-awareness and be able to use a self-care program. I am here on this or any subsequent occasion strictly for the purpose of obtaining information and guidance on matters related to natural healing and how biofeedback, nutritional supplementation, homeopathic remedies, herbs, diet and lifestyle modification, vibration balancing, meridian energy balancing, and removing toxins can in theory, may achieve better health.

I understand that no recommendation, discussions, inclusion/sale of nutritional supplements, or homeopathies will be made. Energetic remedies, called “Drops,” pertain to the “whole body” ENERGETIC CONCEPT and do not relate to the context of any specific ailment, nutrition, or condition.
